Commonwealth Home Support Program

Commonwealth Home Support Program (CHSP)

OT Services Group is funded by the Australian Government - Department of Health, Disability and Ageing to deliver occupational therapy services to older adults who are CHSP clients.

The Commonwealth Home Support Program helps older Australians access entry level support services to enable them to live independently and safely at home.

We understand that staying at home means everything. Our CHSP services help you to stay safe, active and independent in your home.

We are funded to provide services in the Perth metropolitan regions (North, East, South East, and South West). We also provide visiting services the South West and Wheatbelt regions.

What can an occupational therapist help with?

Our CHSP occupational therapy services focus on helping older adults manage everyday activities safely and independently. Our Occupational Therapists will assess how a person completes these activities and then identify practical ways to reduce risks and improve function.

​Support may include:
  • Home safety assessments
  • Recommendations for rails or minor modifications
  • Equipment advice and prescription
  • Strategies for everyday tasks like dressing, showering, and meal preparation
  • Falls prevention planning​

How to access our services?

If you already have a CHSP code for occupational therapy services, you can refer directly to our services.
Steps to Refer:
  1. Get your CHSP Referral Code: To access CHSP services, ensure you have a CHSP referral code specifically for occupational therapy. You will receive this from My Aged Care following your Aged Care Assessment.
  2. Contact us directly: Reach out to us by phone or through our website to provide us with your CHSP referral code. Our support team will guide you through the referral process and ensure we have everything needed to start providing services.
If you don’t yet have a CHSP code, contact My Aged Care (1800 200 422) directly for eligibility and assessment.

​What are the costs for CHSP services from OT Services Group?

The Australian Government - Department of Health, Disability and Ageing provides the bulk of the funding for CHSP services. However, they ask that people using the services contribute to their cost, to help the CHSP be financially sustainable.

All clients who can afford to contribute to the cost of their care should do so. At OTSG contributions are applied consistently across the client base, with clear categories. OTSG charges a flat rate contribution using a sliding scale based on the client’s financial position. The contribution is charged at the time of initial OT assessment. If you are assessed as unable to pay, you will not be denied services or have them discontinued. Aged Pension

(Pensioner Concession Card holder)

We exempt fees for Pension Concession Card holders to support low income households, reduce financial barriers to essential services, and promote fairness for vulnerable community members.

DVA Veteran

(DVA Gold Card / All Conditions holder)


Exempt


We exempt fees for Veteran Gold Card holders to honour their service, express our gratitude, and support the health and well-being of our veteran community.

FAQs

How do I know if my chsp package covers occupational therapy services?
​CHSP can fund occupational therapy services when you have been assessed as eligible through My Aged Care and have a referral for Allied Health and Therapy Services - Occupational Therapy.

You will receive a Notice of Decision Letter and Support Plan that will outline the services you have been approved for. If you are unsure, you can contact My Aged Care or our team and we can help you check what referral information is needed.

Do i need a referral?
Yes. To access any CHSP funded service you need a referral code. You can contact My Aged Care directly, or speak with our team if you are unsure where to start.
What is the difference between chsp & home care/ SUPPORT AT HOME packages?
​CHSP provides entry level support for older people who need a small amount of help to remain living safely at home. Home Care Packages have now transitioned to the Support at Home program, which provides a higher level of ongoing support for people with more complex or regular care needs.
Will I need to pay anything?
 CHSP services are subsidised by the Australian Government, but you may be asked to pay a client contribution (see our Client Contribution Policy). Your therapist will discuss any fees with you before services start. ​However, if you are assessed as unable to pay, you will not be denied services or have them discontinued.
What information do I need before booking?
To book directly with our service - contact us with your My Aged Care referral code, your contact details, and a brief summary of what you would like help with. If you have been approved for CHSP services, your referral code can be found in your My Aged Care information, and we use this code to connect your referral to our service.
Can I choose OT Services Group as my provider?
Yes.

Where we provide the relevant service in your geographical area and have availability we can provide you with services. Once you have been approved for CHSP, you can ask My Aged Care to send your referral to your preferred provider, or you can provide your referral code directly to the provider.

Can CHSP help with equipment or home modifications?
CHSP may support access to services that help you remain living safely at home, including allied health, therapy and some home modification or equipment related supports. What is available depends on your My Aged Care assessment, approved referral codes, local service availability and funding pathway.
What if my needs have changed?
If your needs have changed, you may need to contact My Aged Care for a support plan review or reassessment. This can help confirm whether your current services are still suitable or whether a different type or level of support is needed.
Do you visit people at home?
Yes. For CHSP occupational therapy, services are usually provided in the home or community environment so the therapist can understand your usual routines, access needs and safety considerations. However, we do also provide clinic-based services in Bentley, and Bunbury to support urgent equipment need assessments.
